2009 Early Childhood Recognition Awards Directions

Sponsored by Mid-York Child Care Coordinating Council, Inc.

The two major areas of award will be Friend of Children and Employer Recognition.

Awardees will be announced and awards presented at the Awards Dinner in June 2008.

I. FRIEND OF CHILDREN AWARD

Purpose: To recognize and honor the dedication of people in Herkimer, Madison and Oneida counties who exemplify and promote the ideal of quality health and safety of children either through direct or indirect service and support to families.

I.a.) Direct Service--An award will be presented to an outstanding individual in the following sub-categories of legal child care:

A. Directors or Assistant Directors of a child care center or school-age child care program

B. Teachers, Assistant Teachers, Teacher's Aides in a child care center or school-age child care program

C. Parent Volunteers, Senior Aides, Students, Interns or other volunteers working with children

D. Family or Group Family Child Care Providers

E. Providers who are legally exempt (such as nursery school or children's camp personnel)

I.b.) Indirect Service--An award will be presented to an outstanding organization or community member who demonstrates support for quality child care services through support and action in each of the following sub-categories:

F. Human Service Agency or Organization that demonstrates their Friend of Children stance through collaboration with and/or action toward the child care community

G. Community Member who has demonstrated excellence and leadership in te promotion, support and advocacy of quality child care

H. Posthumous

I. Youth (nominee must be 8 to 17-years-old)

II. EMPLOYER RECOGNITION AWARDS

Purpose: To recognize and honor an employer that has demonstrated leadership in establishing family-friendly policies or services for their employees with young children.

To nominate an employer in the above category, please complete the attached nomination form AND

> Describe the employer's most significant accomplishments including, but not limited to, dependent care tax incentives, benefits, flex hours, family leave time, etc.

> Describe those attributes that would encourage other employers to view the candidate as a role model (please list specific examples)

When completing a nomination form, please remember that answering every question is essential, as the judges assign a point value to each answer and if a question is skipped, it reduces the maximum number of points a nomination can earn. Points are assigned within the range that is assisted within the range that is in parenthesis behind each question. When responding to the questions, remember to provide the judges with as much detail as you can because all they know about the candidate is what you tell them!

For nominations of Direct Service Providers only, please attest: The individual that I am nominating is affiliated with a legal mode of child care within Herkimer, Madison or Oneida county, including a licensed child care center, licensed group family day care home, registered family day care home, or school-age child care program regulated by the NYS Office of Children and Family Services, a program regulated by the NYS Department of Health (such as a children's camp) or legally exempt care (such as a nursery school or child care for 2 or less unrelated children in a home setting).

For nominations of direct service providers, community members, youth or agencies, please complete the following four questions:

1. Please describe specific reasons why you feel the nominee stands out in professional or volunteer service. List any community or advocacy involvement related to children and families or ways in which the nominee goes "above and beyond" (Point value: 1--10).

2. Please list any personal/professional challenges overcome or improvements made that have had a
positive effect on children's lives. (Point value: 1--10)

3. Describe at least one postive change in your child or the children in your community that you would credit directly
to this nominee and explain why. (Point value: 1--10)

4. Specify other reasons why the nominee should be considered for this award ((Point value: 1--5).
